Best case scenario: The warhead falls off the missile and lands intact and essentially inert. Worst case scenario: the conventional explosives that trigger the nuclear detonation go off, scattering the the plutonium over a wide area. In this case Putin would just remove people until he got someone to follow the order. Possibly a full revolt of his entire high ranking military personnel would stop him....but I wouldn't bet on it. Because a coup is what it would take to stop it for sure and a man like Putin may see such a move by generals(even if they made clear that was the only order they would refuse) as tantamount to a coup.
