Absolutely this. We had a prisoner with COPD a few years back, he literally had to take multiple breaks to walk about 200 feet to the cafeteria from his cell, dragging oxygen.

Nyneave is older, but rand, Matt, and Perrin are not 19. The series covers about 3 years, and they are barely 20 when it finishes. None of the boys or Egwene are adult when the story starts.

And again, the characters are 16 and 17 when when the books start. I can't imagine how anyone expected different.

The series literally starts with characters who are like 16 years old. I don't know what you expected. Rand is like 20 when the whole series finishes. 

I actually can't complain, except for maybe Perrin. Not because he's black, I don't give a shit about that. But because perrin is supposed to be a big, muscular blacksmith who swings a huge hammer. Otherwise, as long as they can act, I think the cast is looking pretty good.

Yes, the Hyoid often breaks. Its a very fragile bone, and it'll break from pressure, not just force. I know people who have broken theirs getting choked in Jiu Jitsu class.
